The Physical Therapist (PT) is responsible for helping patients achieve their treatment goals by administering therapeutic exercises and activities, both manual and non-manual, and assisting with functional mobility and daily living tasks. The PT evaluates and interprets clinical assessments, collaborates with physicians to develop individualized care plans, and ensures the delivery of high-quality, compliant care.

Responsibilities Include:

• Conduct Start of Care (SOC) visits
• Perform comprehensive evaluations and develop treatment plans 
• Provide skilled physical therapy services to Medicare and VA patients 
• Accurately document all patient care in the electronic health record AXXESS 
• Maintain compliance with federal, state, and professional regulations and standards 
• Communicate effectively with physicians, patients, families, and care team members 
• Monitor and report on patient progress, making adjustments to plans as needed

Qualifications:

• Start of Care (SOC) - OASIS experience is required 
• Recent PT experience
• Current PT License in CA
• Current CPR card
